個人資料蒐集告知暨同意書


國立清華大學「環安衛E化管理系統」(以下簡稱本系統)為蒐集、處理、利用個人資料,依個人資料保護法之規定以本聲明向您告知並徵求您同意,請您詳閱。

一、 蒐集個人資料之目的及內容

(一)

本系統基於運作及管理之目的,須蒐集您的「身份證號、出生年月日、姓名、單位、職稱、E-mail、電話」等個人資料,作為資料建置、管理、公務人員學習時數登入、統計分析及必要聯繫之用。

(二)

您的資料將永久保存,以便系統未來不定期提供您相關訊息。

(三)

本系統將自校方人資系統蒐集使用者個人資料,並僅作系統內部使用。

(四)

本系統將進行各項網路資源服務之管理及記錄,包括蒐集IP位址、瀏覽網頁、使用檔案及時間等軌跡資料,並僅作系統內部使用。

二、 個人資料之提供

(一)

請依各項服務需求提供您本人正確、最新及完整的個人資料,若您的個人資料有任何異動,請主動申請更正。

(二)

您可自由選擇是否提供相關個人資料,惟若拒絕提供個人資料,本系統將無法提供相關服務。

(三)

您得依中華民國「個人資料保護法」第三條規定,就您的個人資料行使以下權利:

									1. 請求查閱、補充、更正。
									2. 請求提供複製本。
									3. 請求停止蒐集、處理、利用。
								
(四)

請求刪除個人資料等權利,請洽系統承辦人員連絡。各項資料如未完整提供,將無法進行本系統運作。

三、 資訊安全

(一)

本系統將嚴密保護您所登錄或留存之個人資料,使其免於損失、遭竊或濫用,並避免遭受未經授權之擅自存取、揭露、變更或損毀。

(二)

為維護您的權益,請妥善保存您的帳號及密碼,如有遺失或被竊之慮時,應立即變更你的密碼,以嚴防個人資料外洩。

四、 同意之效力

(一)

當您簽署本同意書時,即表示您已閱讀、瞭解並同意本同意書之所有內容。

(二)

本系統保留隨時修改本同意書規範之權力,並將於修改規範時,於本系統公告修改之事實,不另作個別通知。如果您不同意修改的內容,請主張停止蒐集、處理及利用個人資料;否則將視為您已同意並接受本同意書該等增訂或修改內容之拘束。

Personal Data Collection Notification and Consent Form


The National Tsing Hua University「Environmental Health and Safety E-management System 」(hereinafter referred to as "the System") collects, processes, and utilizes personal data in accordance with the Personal Data Protection Act. This statement informs you of the purposes and seeks your consent. Please read carefully.

I. Purpose and Content of Personal Data Collection

For operational and management purposes, the System collects your personal data including "ID number, date of birth, name, department, job title, E-mail, phone number," etc., for data establishment, management, civil servant learning hour login, statistical analysis, and necessary communication.

2.

Your data will be permanently stored for future periodic provision of relevant information by the system.

3.

The System collects and uses user personal data from the university's human resources system for internal system use only.

4.

The System manages and records various internet resource services, including collecting IP addresses, browsing web pages, using files, and timestamps, for internal system use only.

II. Provision of Personal Data

1.

Please provide your accurate, up-to-date, and complete personal data according to the service requirements. If there are any changes to your personal data, please apply for corrections proactively.

2.

You are free to choose whether to provide relevant personal data. However, if you refuse to provide personal data, the system will be unable to provide related services.

3.

According to Article 3 of the Personal Data Protection Act of the Republic of China, you have the following rights regarding your personal data:

									3.1. Request to access, supplement, or correct.
									3.2. Request to provide a copy.
									3.3. Request to stop collecting, processing, and using.

For rights such as requesting deletion of personal data, please contact the system administrator. Incomplete data provision will impede the operation of the system.

III. Information Security

The System strictly protects the personal data you register or store from loss, theft, or misuse, and prevents unauthorized access, disclosure, alteration, or damage.

2.

To safeguard your interests, please keep your account and password secure. In case of loss or theft concerns, change your password immediately to prevent personal data leakage.

IV. Effectiveness of Consent

By signing this consent form, you acknowledge that you have read, understood, and agreed to all the contents of this consent form.

The System reserves the right to modify the regulations of this consent form at any time and will announce the modifications on the system without separate notice. If you do not agree to the modified content, please assert to stop collecting, processing, and using personal data. Otherwise, it will be deemed that you have agreed to and accepted the binding of such additions or modifications to this consent form.
